Your registration is straightforward — complete it at home, online, or right here in our office. Whatever is easiest for you.
Getting started as a new patient here is simple — we have kept it that way on purpose. You can fill out your forms online before your visit, print them and complete them at home, or just arrive a few minutes early and we will walk you through everything in person at our East Main Street office.

Complete your new patient forms online ahead of time, print them at home, or fill them out in our office — whichever you prefer.
Bring a Few Things
Your photo ID, insurance card (if you have one), and a list of any medications. We take care of everything else.
